The reply card asks only for what we need to call you back: the kind of visit, a good time, your name, phone and, if you like, email. It does not ask about your health, and please do not send health details through this site or by email. We go over your medical history privately, in person.

What you send goes to our front desk over an encrypted connection and is used only to arrange your consultation. We do not sell it or share it with advertisers, and this site does not use advertising trackers.

The tick list in Chapter one runs in your browser and is never sent anywhere.

Once you are a patient, your records are handled under our HIPAA Notice of Privacy Practices, available at the front desk. Owner: link your Notice of Privacy Practices
